Dreher lives in Amherst with her life partner. Dreher's mother was of Welsh descent and instilled in her daughter a love for theatre through regular trips to New York to see shows. In the mid-1950s Dreher attended Wellesley College, studying clinical psychology.

It was at college that she began to be aware of her sexual identity, trying to, as she put it, "come to grips with being a lesbian in a homophobic world". She had the experience of being called to the Dean's office because another woman had accused Dreher and a friend of being in a lesbian relationship.

Dreher is particularly well known for her lesbian detective novels centering on lesbian butch sleuth Stoner McTavish, that include Stoner McTavish (1985), Something Shady (1986), Gray Magic (1987), A Captive in Time (1990), Otherworld (1993), Bad Company (1995) and Shaman's Moon (1998).
